REQUIREMENTS (All 9 items are required for the application to be considered.)

 

-An Application fee of $10.00

-A printed passport size photograph

-Official transcript from school (Standard 4, 5 and 6) –  Do not submit report cards

-Principal’s recommendation in sealed envelope (A thorough description of conduct and academics)

-Teacher’s recommendation in sealed envelope. (A thorough description of conduct and academics)

-A photocopy of your child’s birth certificate (preferably the new one)

-A photocopy of your CHILD’s VALID social security card.

-A photocopy of MOTHER’s or FATHER’S VALID social security card.

-A personal statement explaining why you want to attend Cornerstone Presbyterian High School.  (A paragraph of at least EIGHT sentences)
